As an After Hours Hospice RN, you’ll respond to patient needs during evenings, nights, weekends, and holidays—bringing peace of mind and compassionate care when it’s needed most.
What You'll Do as a Hospice RN – After Hours / On-Call:
• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
• Provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care
• Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
• Accurately document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and coordination with the daytime care team
• Assures the highest quality of care is provided and documented in accordance with the Plan of Care.
• Performs the initial and on-going assessment of the patient.
• Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group.
• Responsible for the education of the patient, family or other caregivers in relation to needs identified on the Plan of Care, including, but not exclusive of, medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient.
• Works closely with other members of the Interdisciplinary Group to provide all-inclusive physical, psychosocial, educational, emotional and spiritual services including bereavement support to the patient and family to achieve the highest quality of care.
• Maintains primary control and professional management of each patient and acts as primary liaison between physician, patient/family and hospice team so as to be fully aware and abreast of all clinical issues, staffing and scheduling concerns.
• Reports any change in the patient’s condition to both the Attending Physician and Hospice Physician.
• Maintains close contact with the patient’s family and/or significant others to provide information, support, and continuity of care.
• Maintains an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, initial and on-going assessments and any other information pertinent to the care of the patient.
• Monitor and maintain continuous access to the TeleMed mobile application on a cellular device and/or tablet during all scheduled shifts to ensure immediate response capability, prompt communication and task execution and completion.
• Performs on-going review and follow up on assigned patient clinical records to assure quality services are provided, standards of practice are followed, and compliance with policies, procedures and regulatory requirements is met and maintained.
• Practices infection control measures and standard precautions.
• Makes routine supervisory visits of, and with, all other team members at least monthly.
• Monitors scheduling of all staff.
• Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process.
• Participates in the Quality Assessment and Performance Improvement Program.
• May be asked to perform routine visits during normal business hours
• Attends staff meetings.
• Performs other activities as assigned.
• Consistently promotes company’s core values.
• Completes required Company annual training.
• This is a safety-sensitive position.
Experience - What you'll bring:
• Current RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification
Preferred Experience (Not Required):
• 1+ years of hospice, home health, oncology, or palliative care experience
• Familiarity with symptom management, triage, and after-hours home-based care
• Prior experience in on-call or overnight nursing roles
• Strong documentation and electronic medical records (EMR) skills
