Join our hospice team as an Afterhours RN Care Manager, where you'll provide compassionate care when patients and families need it most. This role is ideal for an experienced RN who enjoys autonomy, critical thinking, and building meaningful connections while supporting patients in their homes during evenings, nights, weekends, and holidays.
As an Afterhours RN Care Manager, you'll ensure patients and families receive exceptional hospice care outside traditional business hours. You'll respond to urgent clinical needs, provide symptom management, coordinate care, admit new patients, and offer compassionate support during some of life's most important moments. You'll work closely with the interdisciplinary team to ensure continuity of care while serving as a trusted clinical resource during after-hours coverage.
This role combines direct patient care with clinical coordination and oversight, allowing you to make a meaningful impact while enjoying the work-life balance of a 7-on/7-off schedule.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range: $84,000 - $94,000
