We are seeking a Hospice Clinical Liaison (LPN/LVN) to represent our hospice program to healthcare facilities such as nursing homes, assisted living communities, and hospitals. In this role, you will be the bridge between patients, families, and care teams—providing education, coordinating referrals, and ensuring a smooth transition to hospice services. This position requires clinical expertise, excellent communication skills, and a passion for building lasting relationships in the healthcare community.
Essential Functions
Knowledge, Skills, and Abilities
Excellent interpersonal and customer service skills.
Ability to work independently while thriving in a team-focused environment.
Strong presentation and communication skills in both formal and informal settings.
In-depth knowledge of hospice regulations, discharge planning, and referral processes.
Proficiency with Microsoft Word, Excel, and CRM/documentation systems.
Willingness to travel within an assigned territory, with occasional overnight stays.
Education
Graduate of an approved school of Practical/Vocational Nursing.
Experience
Minimum two years’ experience as an LPN/LVN in hospice, home health, oncology, or long-term care.
Three to five years of clinical liaison or healthcare marketing experience preferred.
Licenses/Certifications
Current LPN/LVN license in the state of practice.
CPR certification.
Valid driver’s license and automobile insurance.
