*This position will require extensive multi-state travel, potentially up to 80% during the week.*

 

We are seeking an experienced and mission-driven Nurse Practitioner Preceptor to join our Palliative Care division. In this role, you will mentor, coach, and train newly hired and tenured nurse practitioners through hands-on patient care and structured clinical education. You will ensure that preceptees are confident, competent, and aligned with our mission of delivering exceptional patient-centered palliative care.

 

As a role model and trusted clinical leader, you will balance mentoring responsibilities with patient home visits, directly impacting both clinical quality and patient outcomes across our Palliative Care/Empatia program.

Essential Functions

  • Assess the learning needs of new and tenured nurse practitioners and establish individualized orientation goals and measurable learning outcomes
  • Orient preceptees to palliative clinical policies, procedures, documentation standards, value-based care metrics, technology, and compliance requirements
  • Facilitate learning experiences to promote the acquisition of clinical skills, knowledge, and professional competencies
  • Provide timely, constructive feedback and ongoing coaching to preceptees throughout their orientation or development period
  • Conduct patient home visits as needed, modeling best practices in compassionate, evidence-based care
  • Evaluate preceptee readiness for independent practice and make recommendations to leadership
  • Monitor key performance metrics, providing targeted feedback and performance improvement support
  • Prepare, maintain, and submit accurate documentation in accordance with company policy and regulatory standards
  • Refer patients to additional services and community resources when appropriate
  • Collaborate with leadership on initiatives to strengthen clinical quality and patient satisfaction

About You

Qualifications

  • Advanced Practice Practitioner (Nurse Practitioner) with an active, unencumbered NP and RN license in assigned work state(s)
  • Active DEA license and current CPR certification
  • Minimum of 5 years clinical experience, including prior experience as a preceptor or clinical mentor
  • Strong knowledge of value-based clinical care and Part B documentation requirements (preferred)
  • Valid driver’s license, reliable transportation, and liability insurance

Skills & Competencies

  • Proven ability to mentor, coach, and inspire peer clinicians
  • Strong presentation and communication skills, both written and verbal
  • Excellent organizational and time management abilities
  • Proficient in EHR documentation, Microsoft Office Suite, and mobile technology
  • Compassionate, patient-focused, and collaborative approach to care delivery
