Coordinate Care. Support Patients. Transform AIM and Palliative Services.
We are seeking an organized and detail-oriented LPN Clinical Coordinator to join our Advanced Illness Management (AIM) and Palliative Care team. Reporting directly to the AIM Clinical Manager, this role coordinates and manages day-to-day clinical operations, scheduling, medical record maintenance, and supports interdisciplinary care. The Clinical Coordinator ensures efficient patient care, adherence to visit frequency, and smooth collaboration across the palliative care team.
Essential Functions of Position:
Coordinate and direct all day-to-day clinical activities for AIM and Palliative Care.
Serve as primary contact for incoming patient and partner calls, ensuring timely follow-up.
Process referrals by collecting admission and insurance information to ensure timely patient admission.
Schedule and maintain efficient visit coordination for Nurse Practitioners and clinical staff, considering geography and time management.
Monitor Risk Scoring and ensure visit frequency adherence as indicated by Global Alerts.
Reschedule and coordinate PRN visits, post-hospitalization follow-ups, and other care needs.
Track ED visits and hospitalizations, gather required documentation, and document in hospitalization tracker.
Support NPs with ordering DME, labs, imaging, and track results to ensure timely delivery to providers.
Coordinate lab drop-off with local partner offices as needed.
Enter communication, updates, and medication reconciliation in partner EMR systems.
Assist staff with credentialing documentation and maintain credentialing logs.
Manage clinical records functions, ensuring compliance with state, federal, and company policies.
Prepare IDT agendas and support interdisciplinary team meetings.
Oversee Home Health and Hospice referrals, ensuring timely processing and acceptance.
Maintain professional relationships with referral sources, monitor satisfaction levels, and identify opportunities for improved services.
Document and follow up on customer complaints in coordination with program leadership.
Participate in quality assurance and performance improvement activities for AIM programs.
Implement marketing and promotional initiatives as directed.
Adhere to HIPAA, Business Ethics, Compliance programs, and all company policies.
