If you’re looking for a unique role that blends social work with community coordination, we’d love for you to join us as a Hospice Social Worker (BSW) – Volunteer Coordinator.

 

In this dual role, you’ll support patients and families through psychosocial care while also leading, training, and advocating for our hospice volunteers. As a Hospice Social Worker – Volunteer Coordinator, You Will:


• Provide psychosocial support to patients and families in accordance with their individualized plan of care
• Conduct social work assessments and provide education, case management, and care planning services
• Offer emotional and resource-based support related to family relationships, cultural/spiritual beliefs, long-term care, and advance care planning
• Monitor and support safe home environments for hospice patients
• Serve as the key liaison and advocate for all volunteer services within your branch
• Assess patient and family needs for volunteer support and assign volunteers accordingly
• Supervise, train, and support volunteers, ensuring files, documentation, and visit logs are current and compliant
• Plan and facilitate ongoing volunteer education, recognition, and engagement

About You

Qualifications – What You’ll Bring:


• Bachelor’s Degree in Social Work (BSW) from a CSWE-accredited school
• Licensure as required by the state of employment
• At least 1 year of experience as a Social Worker in a healthcare or hospice setting
• Ability to apply social work principles to end-of-life care, including understanding cultural, emotional, and spiritual dynamics
• Strong interpersonal skills, emotional intelligence, and the ability to support others during times of grief and transition
• Organizational skills to manage volunteer coordination, documentation, and program engagement

 

Preferred Experience:


• 3 to 5 years of hospice or healthcare social work experience
• Prior experience supervising or coordinating volunteer programs
• Familiarity with community resources and long-term care planning
• Training in bereavement or crisis counseling
