We are seeking a dedicated Continuous Care Licensed Practical Nurse (LPN) to join our team. Reporting to the Executive Director/Administrator or Director of Clinical Services, and working under the direction of a Registered Nurse, you will provide continuous nursing care to hospice patients in facility or home settings. Your role is vital in monitoring patient conditions, managing acute symptoms, and supporting families during critical times.
Key Responsibilities
Provide skilled bedside nursing care following the patient’s plan of care and under RN supervision.
Monitor patients actively, report changes in condition, and manage acute medical symptoms to achieve palliation during crisis periods.
Administer medications, monitor vital signs, and evaluate patient and family needs.
Assist with plan of care preparation and execution, including infection control and symptom management.
Document patient care activities and communicate findings with interdisciplinary teams and physicians.
Participate in bereavement support and may assist with death pronouncements as allowed by state law.
Provide guidance to hospice aides under RN supervision as per Nurse Practice Act regulations.
Participate in call schedules and provide on-call nursing support as directed.
Qualifications
Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state of employment.
CPR certification required.
Valid driver’s license and current automobile insurance.
Graduate of an accredited Practical Nursing program.
Minimum of 2 years’ experience as an LPN/LVN in hospice, home health, or long-term care; 3–5 years preferred.
Work Environment
Care is provided in patient homes or institutional settings with travel required.
Exposure to infectious diseases, bodily fluids, and hazardous materials possible.
Skills and Knowledge
Strong knowledge of hospice patient needs, terminal illness care, and interdisciplinary team roles.
Excellent patient assessment and communication skills.
Understanding of regulatory guidelines such as Medicare, Medicaid, and ACHC.
Ability to provide compassionate support to patients and families during end-of-life care.
Effective documentation and program management skills.
Personal Attributes
Compassionate and sensitive to the emotional needs of terminally ill patients and their families.
Flexible and adaptable, able to manage multiple tasks efficiently.
Professional, mature, and cooperative with strong interpersonal skills.
Tools and Equipment
Proficient with nursing assessment tools, computers, fax, copier, and communication devices.
Reliable transportation required for travel between patient locations.
