Overview

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as an AIM Clinical Coordinator. This position plays a vital role in supporting the Advanced Illness Management (AIM) Palliative Care program.

 

You’ll be responsible for managing day-to-day clinical operations, scheduling, and care coordination, ensuring seamless delivery of high-quality, patient-centered care.

 

This role reports directly to the Clinical Manager, and serves as a clinical and operational bridge between patients, providers, and our healthcare partners.

 

Key Responsibilities:

  • Coordinate and manage all daily clinical activities for the AIM and Palliative Care departments
  • Schedule and ensure visit frequency adherence, including PRN visits and post-hospitalization follow-ups
  • Serve as the primary point of contact for patient and partner inquiries
  • Process patient referrals, ensuring accurate insurance and admission documentation
  • Order and track labs, imaging, and DME; ensure timely result delivery and documentation in EMR
  • Support Nurse Practitioners by entering communications and medication reconciliation in partner EMRs
  • Identify Home Health and Hospice-eligible patients and coordinate timely referrals
  • Maintain and organize clinical records in accordance with federal and state regulations
  • Track and manage ED visits and hospitalizations; schedule post-discharge visits
  • Coordinate interdisciplinary team (IDT) meetings and maintain agendas
  • Support credentialing processes by submitting necessary documents and maintaining logs
  • Partner with local labs for specimen drop-off coordination
  • Engage in quality assurance, performance improvement, and marketing initiatives

About You

  • Registered Nurse (RN) license in good standing
  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred
  • 3+ years of experience in healthcare delivery, care coordination, or palliative/home health settings
  • Experience with Part B billing and knowledge of insurance reimbursement processes preferred
  • Proficiency in Electronic Medical Records (EMR) and Microsoft Office tools (Outlook, Word, Excel)
  • Strong understanding of medical terminology, risk scoring, and care transition best practices
  • Excellent written, verbal, and interpersonal communication skills
  • Detail-oriented with the ability to manage multiple tasks and shifting priorities
