You’ll report directly to the Executive Director or Administrator and be assigned to a location to perform Face-to-Face (F2F) visits. In this role, you will assess patients and provide detailed clinical documentation to support the physician’s certification or recertification of hospice eligibility—based on CMS regulatory standards. Your assessments help ensure that patients continue to receive the compassionate care they need at the end of life.

As a Hospice Nurse Practitioner, You Will:
Perform and document physical assessments for Face-to-Face (F2F) hospice recertification
Review diagnostic tests, treatment plans, medication lists, and lab results to support hospice eligibility
Communicate clinical findings to the certifying hospice physician
Make informed recommendations to the Interdisciplinary Group (IDG) based on your F2F evaluations
Notify the agency promptly when F2F visits cannot be completed as scheduled
Ensure full compliance with CMS timelines and hospice documentation standards
Requirements:
Master’s or higher degree in Nursing
Completion of a graduate-level nurse practitioner program accredited by a national body
Current RN license in the state of requested employment
Current state license or certificate as an Advanced Practice Registered Nurse (APRN)
National certification as a Nurse Practitioner (ANCC or AANP)
Active collaborative agreement in place, as required by state law
Preferred Experience:
Previous experience in hospice, palliative care, geriatrics, or home health
Familiarity with Medicare regulations and hospice eligibility criteria
Experience performing F2F or recertification visits
Ability to work independently and manage a flexible visit schedule
Strong communication and clinical documentation skills
