Deliver comfort and dignity - one patient at a time
We’re looking for a Hospice LPN to join our team. You’ll report directly to the Administrator or Executive Director and play a key role in providing compassionate, hands-on care to patients and their families. This position supports end-of-life care in the setting patients call home—whether that’s a private residence, assisted living, or facility.
As a Hospice LPN, You Will:
Provide direct patient care services to hospice patients and supportive care to families with a reasonable caseload
Assist the interdisciplinary team in evaluating patient/family needs and contribute to the plan of care
Report findings or changes in the patient’s condition to the Registered Nurse
Deliver care with empathy and attentiveness in accordance with the patient’s individualized care plan
Serve as a valued resource to families, helping them feel supported during difficult moments
Requirements:
Current LPN/LVN license to practice in the state of requested employment
At least 1 year of direct patient care experience in hospice, home health, ICU, geriatric, oncology or other high acuity setting
Current CPR certification
Current driver’s license and automobile insurance
Hospice heart: compassionate, kind, and empathetic to patients and families
Comfortable providing care in diverse home settings
Able to demonstrate flexibility and composure while supporting others during crisis
Preferred Experience:
Three or more years of clinical nursing experience, especially in hospice or palliative care
Background in hospital settings such as Med-Surg, PACU, ER, Float Pool, Critical Care, or Acute Care
Familiarity with telemetry, wound care, admissions, cardiac care, or nursing home environments
Travel nurse or mobile healthcare experience
