Key Responsibilities:

  • Serve as the primary point of contact for new patient referrals
  • Initiate outreach to patients and families within 1 hour of referral
  • Coordinate all admissions within the company’s 3-hour commitment window
  • Verify insurance benefits, payer source, and secure authorizations for hospice care
  • Communicate pending admissions to appropriate clinical and admission staff.
  • Collaborate with clinical teams, billing coordinators, and external referral sources
  • Enter accurate patient data and documentation into the electronic system
  • Participate in morning stand-up meetings, marketing activities, and outreach including seeking new referrals sources and developing strategies for company growth
  • Prepare marketing reports regarding referral statistics.
  • Maintain communication with pending and Not Taken Under Care (NTUC) patients
  • Support weekend and on-call admissions as needed

About You

  • High school diploma or equivalent required
  • LPN/LVN license or bachelor’s degree in nursing, Business, Psychology, or Social Work strongly preferred
  • Previous experience in customer service
  • Hospice or medical office experience preferred
  • Strong skills in communication, organization, negotiation, public relations and data entry
  • Familiarity with Microsoft Office and healthcare systems
  • Knowledge of hospice admission regulations, compliance, and insurance processes is a plus
