Overview

We are looking for an Afterhours Coordinator to join our team. This position will directly report to the Manager Afterhours Clinical Support and is responsible for supporting the After Hours team in the processing of After Hours work flow, as well as supporting the on call RNs. This role is under established performance criteria.

  • Responsible for specific HCHB workflow, visit scheduling, as well as to support referral coordination for patients who need Gentiva Services.
  • Reviews medical records against nationally recognized clinical criteria guidelines using clinical decision making to determine the eligibly for other levels of care.
  • Adheres to and participates in Company’s mandatory training which includes but is not limited to HIPAA privacy program/practices, Business Ethics and Compliance programs/practices, and Company policies and procedures
  • This is a work from home telephonic position.

About You

  • Minimum of 1 year working in a healthcare related field or healthcare call center
  • INTERNET REQUIREMENT: High-speed Internet connection (minimum 10 Mb/s download speed and minimum 2 Mb/s upload speed, recommended 5 Mb/s upload). Satellite based internet services are not acceptable
  • Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills
  • Excellent analytical and problem-solving skills
