Overview

We are looking for a Centralized Intake Coordinator to join our team. This position will directly report to the Manager of Centralized Intake and is responsible for overseeing that all referrals are sent via fax, telephone, email and through an electronic platform. This includes oversight and timely response to all referrals issued by subscribing facilities and management and implementation of all aspects associated with acceptance and/or response to eReferrals and coordination with the local Gentiva family brand.

 

The shift is Friday, Saturday, Sunday 8am-5pm, part-time.  

About You

  • High school diploma or equivalent with customer service experience; Bachelor’s degree in business, social work, psychology or nursing or equivalent business experience preferred
  • Must have excellent professional communication
  • Excellent organization skills
  • Able to multi task
  • Hospice or medical office experience preferred
  • LPN/LVN certification preferred
