Overview

We are looking for a Registered Nurse PC to join our team.  This position will directly report to the Assistant Director PC, Branch Director PC or Branch Manager PC and is responsible for providing skilled nursing care including skilled assessment and instruction in the home under a plan of care/treatment.

  • Confers with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provides hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision.  Revises plan in consultation with physician based on ongoing assessments and as required by policy/regulation
  • Admits patients to the services, completing appropriate clinical documentation under the Clinical Manager's direction. Assesses individual status and care/instruction needed by the patient
  • Coordinates/oversees/supervises the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitors the appropriate completion of documents.

About You

  • Bachelors / Associates degree in Nursing or equivalent.
  • RN licensure
  • Proof of current CPR certification.
  • Six months' of professional nursing as a Registered Nurse.
